A lead capture form on your website creates the lead itself. The enquirer's answers are attached to it, the source is recorded as Form, and the lead is created in the leftmost stage of your board.
One public lead capture form is included with the Leads add-on. The other form templates, and any additional forms, require the Forms add-on.
Creating the form
There is no route to this from the Leads board. Go to Forms in the main menu and open System templates. Under Lead Capture, select Use template on Simple Lead Capture, which arrives with its sections and questions already built.
Onboarding templates alongside it are labelled "Requires Forms add-on".
Editing works the same as any other form. Adding and changing sections and questions is covered in Building your form.
A lead capture form is always public, and its visibility toggle is fixed that way, because submissions come from people who do not have a Socket login.
Mapping answers to lead fields
Opening the form shows a banner confirming that public submissions will create a lead in your pipeline.
Each question carries a Maps to lead field picker. A mapped answer is written to that field on the lead. An unmapped answer is kept as part of the form response, where it has to be read rather than filtered or reported on.
Contact name and contact email are the two mappings the rest of the form depends on, because they populate the lead's contact record and drive the Calendly pre-fill described below.
Question types that affect the lead
Two question types do more than record an answer.
Email address. The address is used four ways: it identifies the enquirer, it is what Socket matches on to avoid duplicate leads, it is what AI lead research uses to find the prospect's website, and it pre-fills a Calendly booking where the redirect is set.
Company Finder. The enquirer searches Companies House and selects their own company, so the registered name and company number arrive without transcription errors, and the client is set up as a limited company.
Calendly redirect after submission
In the form's settings, under Redirect after submission, paste a Calendly link. On submitting the form the enquirer arrives on the booking page, so the discovery call is booked in the same session.
Socket recognises a Calendly link and uses the mapped contact fields to pre-populate the booking, so the name and email are not asked for twice.
Embedding the form on your website
The form embeds directly into your own page, either inline or opening as a popup from a button, so the enquiry page keeps your branding. Copy the snippet from the form's share screen.

What happens when someone submits
A lead is created in the first stage of your board with the answers attached, recorded against the Form source. The form's responses list links through to the lead each submission created.
Submissions are matched on email address. Where the address matches an existing lead, the submission is added to that lead as a note instead of creating a second one. A prospect who submits the form twice, or returns months later, stays on one record with the history together.
Where the company is already a client
Submissions are also matched on the company. Where the enquirer selects a company Socket already holds, the lead is opened against that existing client rather than creating a duplicate.
An existing client enquiring about additional services therefore arrives on your board attached to their own record, with their history and services already in place.
